In this case study, we introduce a male patient who was 65 years old and admitted to the hospital due to experiencing a headache and abnormal mental behavior for one day. After thorough examination, he was diagnosed with acute stroke caused by the rupture of a vulnerable carotid plaque and the formation of a free-floating thrombus.
