肝外胆管上段肿瘤超声检查及ICCEUS结果分析

Tumor in the upper segment of the extrahepatic bile duct. Conventional ultrasound shows unclear liver gate structures and cannot determine if the left and right hepatic ducts are connected. Due to obvious dilation of the left intrahepatic bile duct, a left intrahepatic bile duct percutaneous transhepatic cholangiodrainage (PTCD) procedure was performed under ultrasound guidance. Intracavitary contrast-enhanced ultrasound (ICCEUS) was conducted through a drainage tube. We found that the contrast agent filled the left intrahepatic bile duct well and partially reached the right hepatic duct. However, compared to the left hepatic duct, the diameter of the right hepatic duct at the liver gate was thin, indicating incomplete obstruction at a narrow site where the left and right hepatic ducts communicate. This suggests that the tumor may have spread to the right hepatic duct.
