Skeletal muscle ischemia-reperfusion (I/R) injury is a common clinical problem that can occur in various scenarios, including peripheral vascular injury, compartment syndrome, revascularization crush syndrome, and tourniquet application in orthopedic surgery. However, effective therapeutic approaches to protect skeletal muscle from I/R injury are currently limited. Our study aimed to investigate the potential of Benzoylaconine (BAC) as a protective agent against skeletal muscle I/R injury and elucidate the underlying mechanism.

Using a rat model of skeletal muscle I/R, we observed that BAC effectively protected the muscle from I/R injury by mitigating oxidative stress and inhibiting apoptosis. Our in vitro experiments using H/R-treated C2C12 cells further revealed that BAC enhanced cell viability while reducing levels of CK, LDH, and MDA, and increasing SOD levels. Moreover, we identified an IF1-dependent pathway that activated the AMPK/Nrf2 axis upon BAC treatment, suggesting that BAC protects skeletal muscle from I/R injury through the IF/AMPK/Nrf2 axis.

Our study provides novel evidence supporting the potential of BAC as a myocyte-protective agent against I/R injury, and we establish a previously unknown mechanism involving the upregulation of the IF1/AMPK/Nrf2 axis in mediating the protective effects of BAC.
