Constructing a multidisciplinary collaboration hospital-wide blood glucose management model involving clinical pharmacists, 300 T2DM patients undergoing elective surgery were randomly assigned to intervention and control groups. After 3 days of hypoglycemic treatment, the intervention group showed a significant reduction in blood glucose levels compared to the control group. The incidence of hypoglycemia and severe hyperglycemia in the intervention group was significantly lower than in the control group, and there was a trend towards reduced postoperative infection rates. The intervention group had significantly higher rates of medication adjustment, individualized discharge planning, and medication adherence compared to the control group.
