Building a multidisciplinary 'medication and care' team involving clinical pharmacists for hospital-wide blood glucose management, a study was conducted on 300 T2DM patients scheduled for elective surgery, randomly divided into intervention and control groups. Results showed that after 3 days of hypoglycemic treatment, the intervention group had significant reductions in blood glucose levels compared to the control group, and the amplitude of daytime blood glucose fluctuations was significantly smaller in the intervention group. The incidence of hypoglycemia and severe hyperglycemia in the intervention group was significantly lower than that in the control group, and there was a trend of reduced postoperative infection rates. The intervention group had significantly higher rates of medication adjustment, individualized discharge plans, and medication compliance than the control group.
