Vital signs: blood pressure 120/80mmHg, pulse 80 beats/min, respiration 18 breaths/min, temperature 36.8°C. Head and neck: The head is normocephalic and atraumatic. The neck is supple. No enlarged lymph nodes. Chest and lungs: The chest is symmetrical. Lungs clear to auscultation bilaterally. Cardiovascular: Regular rate and rhythm. No murmurs, gallops or rubs. Abdomen: Soft, non-tender, non-distended. No hepatosplenomegaly. No ascites. No shifting dullness. Bowel sounds are normal. Genitourinary: Normal female external genitalia. No masses, tenderness, or discharge. Rectal: Normal sphincter tone. No masses, tenderness, or blood. Extremities: No cyanosis, clubbing, or edema.
Assessment and plan:
