Carotid vulnerable plaque is a serious health concern that increases the risk of stroke in patients. In this case study, we present the case of a 56-year-old male patient who was diagnosed with a ruptured carotid vulnerable plaque and free-floating thrombus formation, which led to acute stroke. The use of contrast-enhanced ultrasound (CEUS) played a pivotal role in identifying this rare cause, which was later confirmed by computed tomography angiography (CTA), magnetic resonance imaging (MRI), and digital subtraction angiography (DSA). The patient underwent carotid artery stent placement under the protection of a distal embolic protection device and was discharged after his condition improved. The early and timely diagnosis made possible by CEUS was crucial for the patient's successful treatment.
