Based on further diagnostic testing, an enhanced ultrasound examination was performed, which showed a slightly hypoechoic plaque with multiple neovascularization within (Figure 1), accompanied by a partial discontinuity of the fibrous cap on the surface of the plaque. The discontinuity was approximately 1.4mm wide and 4.0mm deep, and was filled with contrast agent (Figure 2). At the proximal end of the plaque, there was a low hypoechoic attachment measuring approximately 7.8×3.2mm, which was attached to the vessel wall, while the distal end was free-floating within the lumen of the vessel (Figures 3 and 4). During the contrast-enhanced ultrasound, the low hypoechoic area did not show any enhancement, and there was slight movement observed.
