Preliminary Investigation Report: Damage to Vessel Structure During Discharging Operation

Based on our on-site investigation and information gathered from our crew, our preliminary conclusion is that the damage to the vessel was caused by the grab impacting the vessel's structure during the discharging operation.

Evidence Supporting Our Conclusion:

1. Location and Nature of Damage: The damaged hole is located on the sloping plate of No. 5 cargo hold, an area only accessible to the grab during operation. Additionally, the grab itself was found to be covered in fuel, further supporting its involvement in the incident.

2. Signs of Physical Impact: The damaged area exhibits clear signs of physical impact, including concave deformation and scratches. These characteristics strongly suggest the damage resulted from a forceful contact event rather than gradual deterioration like corrosion.

Further Investigation:

While the evidence strongly suggests the grab's impact caused the damage, we are committed to conducting a thorough investigation. We are collecting additional documentation and will perform a comprehensive inspection to definitively exclude other potential causes, such as corrosion.
