1. Introduction
At approximately 2000hrs on July 18, 2023, our surveyor was instructed to attend to a reported incident at the coke terminal. This report details the findings of our investigation into the fuel contamination incident in cargo hold #5 of the subject vessel.
2. Summary of Events
- At approximately 1643hrs on July 18, a grab collided with the sloping plate of the No. 2S fuel tank, causing fuel leakage into cargo hold #5.- The vessel's crew took immediate action to control the leak, adjusting the trim and transferring fuel to minimize spillage.- Upon arrival at 2110hrs, our surveyor interviewed the crew and terminal staff, inspected the vessel and cargo hold, and documented the damage and contamination.
3. Findings
- Cause of Incident: Terminal surveillance video, not provided to our surveyor, reportedly shows the grab colliding with the fuel tank, causing a breach.* Extent of Damage: A 150mm hole was observed in the starboard side hatch coaming and sloping plate of hold #5.* Cargo Contamination: Approximately 30 square meters of cargo directly beneath the hole was contaminated by fuel. The exact quantity of affected cargo is currently unknown.* Environmental Impact: No oil was spilled into the sea.* Terminal Responsibility: While verbally acknowledging responsibility, the terminal refused to sign the stevedore damage report and LOP from the vessel. They also failed to report the incident to the MSA and their liability insurance company.
4. Actions Taken
- The vessel's crew successfully contained the fuel leakage by adjusting the vessel's trim and transferring fuel to another tank.- Bunker operations were temporarily halted and then resumed after the leak was stopped.- The terminal has initiated discussions with a repair company. However, no specific plans for handling the contaminated cargo have been presented.
5. Recommendations
- Unloading of Unaffected Cargo: To prevent further contamination, the unaffected cargo in hold #5 should be unloaded as soon as possible under strict supervision.* Disposal of Contaminated Cargo: The terminal must develop and implement a safe and environmentally sound plan for the disposal of the fuel-contaminated cargo.* Vessel Repair: The vessel's damaged fuel tank should be repaired following proper cleaning and gas-free testing procedures.
6. Next Steps
Our surveyor will remain on board the vessel to:
