Hatch Cover Rack Derailment and Damaged COG Wheel on Starboard Side
The rack of the No. 3 starboard side hatch cover was found dislodged from its track. It was resting on top of the COG wheels. Further inspection revealed a piece of the COG wheel was tilted towards the stern. This incident suggests a potential malfunction in the hatch cover's operating mechanism and damage to the COG wheel assembly.
