Accident Investigation Report: Challenges and Findings

After the incident, our surveyor diligently collected as much information as possible from the scene and made efforts to obtain relevant information through various channels and sources. Unfortunately, due to the ongoing pandemic, we were not granted permission to board the vessel for investigation. Additionally, the foreign crew members have been protesting and showing a lack of cooperation with our investigation.

The captain, who claimed to have recently joined the ship, had limited knowledge of the situation. He was preoccupied with appeasing the foreign crew members and, as a result, did not provide us with the requested documents. Despite our repeated attempts, we have been unable to obtain the necessary documents from the shipowner as well.

However, subsequent to the incident, we came across an article published by an MSA officer on a public website, which provided some insights into the accident investigation. Taking into consideration the content of the article and our own findings at the scene, we would like to address the concerns raised by the association as follows:
