替代性创伤：定义、症状及与相关概念的辨析

什么是替代性创伤？

替代性创伤（Vicarious Traumatization，VT）的概念最早由 McCann 和 Pearlman 在 1990 年提出，用于描述心理治疗师因长期接触遭受创伤的来访者，而自身产生的类似创伤反应。它指的是专业助人者由于持续暴露于他人的创伤经历中，导致自身在认知、情绪和行为上发生负面变化。

替代性创伤的表现：

认知方面: 对自我认知的改变；对诸如安全、信任和控制等重要问题的世界观变化；以及对精神信仰的变化。（Pearlman, 1998; Pearlman & McCann, 1995; Pearlman & Saakvitne, 1995）* 情绪方面: 出现焦虑、抑郁、恐惧、愤怒、悲伤等情绪，以及麻木、疏离、易怒等情绪反应。* 行为方面: 出现失眠、做噩梦、回避与创伤相关的刺激、过度警觉、人际关系问题等。

与替代性创伤相关的概念辨析：

许多学者将替代性创伤和其他相关术语互换使用，如倦怠（Burnout，BO）、同情疲劳（Compassion fatigue，CF）、继发性创伤应激障碍（Secondary traumatic stress disorder，STSD）、创伤后应激障碍等。这些概念之间存在一定的交叉，但并不能等同。

倦怠: 与工作环境和缺乏个人/组织支持有关，表现为情绪耗竭、人格解体/去人格化和个人成就感降低。* 继发性创伤应激: 与 PTSD 症状相似，强调外在的行为症状，如创伤性回忆、回避、情绪麻木等。* 同情疲劳: 由继发性创伤应激和职业倦怠症状组成，强调情绪和身体的疲劳体验。

替代性创伤与上述概念的区别:

替代性创伤更侧重于认知方面的变化，即对自我、世界和他人的看法发生负面改变，而其他概念更侧重于情绪和行为方面的症状。* 替代性创伤可以是即时性的发作，而倦怠和同情疲劳则是随着时间的推移累积发生的。
