Objectives: The objective of this study was to investigate the relationship between cytokines and the outcomes of expectancy management in female patients with primary unexplained infertility (UI) and develop a predictive score for predicting an unsuccessful outcome of expectancy management.

Methods: Female patients with newly diagnosed primary UI who underwent expectancy management were consecutively recruited. After a two-year follow-up via telephone to assess the status of procreation, propensity score matching (PSM) was utilized to form two groups: a fertility group and an infertility group. The levels of plasma IL-2, IL-4, IL-6, IL-10, TNF-α, and IFN-γ were measured using ELISA. Predictive scores were developed using the coefficients of logistic regression of the outcomes of expectancy management with the variables being the levels of the seven cytokines. The performance of the score was validated using 1000 bootstrap resamples.

Results: Eighty-six female patients with primary UI were equally separated into the fertility group and the infertility group. IL-4 (OR=2.60, 95%CI: 1.15, 5.91), IL-10 (OR=0.58, 95%CI: 0.35, 0.95), IL-17a (OR=0.13, 95%CI: 0.03, 0.63), and IFN-γ (OR=14.10, 95%CI: 2.87, 69.28) were identified as predictors of an unsuccessful outcome of expectancy management. Predictive score formulas for an unsuccessful outcome were developed. The area under the curve (AUC) of the receiver operating characteristics (ROC) curves was 0.73 (95% CI [0.62, 0.82]). Bootstrap analysis indicated that the positive predictive values of predictive scores >4.83 in our patient population were 0.95 (95% CI [0.86, 1]).

Conclusion: Our study suggests that cytokine levels can be used to predict the likelihood of unsuccessful expectancy management in female patients with primary UI to a moderate extent. However, the predictive scores need further evaluation for external validity
