In this retrospective cohort study, we investigated the association between inflammatory markers and gestational diabetes mellitus (GDM) in pregnant women. We observed that an elevated systemic immune-inflammation index (SII), neutrophil-to-lymphocyte ratio (NLR), lymphocytes, neutrophils, and platelets were positively associated with an increased risk of GDM during the first trimester. Similarly, these associations remained positive during the second trimester, with stronger associations observed for neutrophils and platelets. The area under the receiver operating characteristic curve (AUROC) revealed that neutrophils and platelets had better predictive power than other inflammatory markers during both trimesters.

In particular, the associations between GDM and neutrophils and platelets were stronger. Additionally, because SII, NLR, lymphocytes, neutrophils, and platelets are readily available in laboratory and clinical settings, they could be proposed as potential predictors for the incidence of GDM.
